The City's health insurance plan is self-funded and administered by CNIC Health Solutions.

EMPLOYEE CONTRIBUTION TOWARD INSURANCE
Individual: $126.64 per month (City pays remaining $440.00)
Family: $302.10 per month (City pays remaining $993.00)
(Includes health and dental coverage)

Family maximum: $1,000
Copayment: 50/50 for costs up to $2,000; 100% thereafter

DENTAL
Individual: $50 per calendar year
Family: $150 family aggregate

There is a minimum 30-day waiting period prior to benefits commencement. Coverage begins the first of the month following 30 full days of employment.

For example:
Date of hire: April 10
30th day: May 10
Coverage begins: June 1

FLEXIBLE SPENDING PLAN

Optional enrollment beginning the first of the month following thirty (30) full days of employment.

Taken out of each paycheck, pre-tax:

The Employee and Family Assistance Program (EFAP) emphasizes prevention and early intervention through counseling and education. Employees and family members who use the counseling office can be assured of receiving confidential assistance from professional counselors.

LIFE INSURANCE

Employees receive life insurance equal to annual salary paid by City to a maximum of $32,000.

Employees may receive another increment equal to annual salary at their cost – 29 cents per $1,000 of salary per month; with optional life insurance; maximum $64,000.
Dependent coverage is available at 63 cents per month. Provides insurance of $2,000 on spouse and $1,000 on each dependent child.

A group insurance policy for short/long term disability is provided by the City at no cost to employees.

PAID LEAVE, HOLIDAYS & VACATION TIME

Paid disability leave: 12 days annually

Holidays: 10 days annually

Vacation accrual rate is based on years of service.

0 through 4 years            10 days annually
5 through 9 years            15 days annually
10 through 14 years         20 days annually
15 through 19 years         22 days annually
20 years or more             25 days annually

Employees begin accumulating vacation from the first pay period. Vacation time can be used during the first year of service with permission of supervisor.
